ARTful Wellness Members can always:

• Opt to freeze your account for up to three months. Your account will resume at the end of your freeze period at the same rate you originally signed up for.

• Cancel at any time. Just call 847-477-8244 before your billing date to stop future payments.

 

The Center for Creative Arts Therapy (C4C) Artful Wellness Member Program (the “Program”) is a program of C4C. The Program is designed to offer benefits to C4C members who commit to a monthly autopay membership. By taking advantage of Program benefits, a Program participant agrees to the terms and conditions for the Program which are set forth below (these “Program Rules”).  We may change these Program Rules; change, add, or eliminate Program benefits, or participating locations; or change or end the Program or any feature of the Program; in whole or in part, at any time, and without notice or compensation, even though such actions may affect rewards or benefits in the Program or your participation in the Program.

Eligible Participants; Enrollment; Continuing Eligibility

​

Eligible participants in the Program are individuals who enroll for a monthly autopay C4C membership using a valid debit or credit card.  As part of enrollment, all required membership documents, agreements and waivers must also be completed and submitted, including any Program forms.  To continue to be eligible in the Program, the participant’s membership and membership account must remain in good standing with C4C at all times, including, without limitation, by not terminating or changing the participant’s C4C membership, or allowing the participant’s C4C membership to be terminated or lapse, and by maintaining payment by autopay for participant’s monthly membership fees.  Once Program eligibility ceases, a member may not take advantage of Program benefits.

Benefits of the Program

​

As of the date of these Program Rules, the standard benefits of the Program are as follows:

​

• Discounted Membership ( ineligible at this time).  Initial eligible Artful Wellness members (i.e., members who have never had an Artful Wellness membership, or other discounted C4C membership) will receive a discounted first month of Artful Wellness membership, at the discount rate provided in the member’s enrollment materials.   A first month discounted membership will not be available to a member again, including for membership lapses, cancellations and subsequent renewals, or other reasons.   The Artful Wellness membership fee for eligible members is a discounted monthly fee (discount and fee as provided in the member’s enrollment materials, and is based on the member’s home market).  There is no activation fee and no minimum commitment.  Prices are subject to applicable taxes, assessments and, as described below, C4C surcharges.  Membership is nontransferable. 

• Freezes and Cancellations.   A member may for up to (but not more than) three (3) months in a calendar year (which may be separate or consecutive months) freeze the member’s Program and C4C benefits; in such event the member will not pay the C4C membership fee for such month(s), during which month(s) the member will not receive Program or C4C benefits, but will maintain the member’s Artful Wellness or other discounted C4C membership, as applicable, member rate. The member’s Program and C4C benefits will reinstate once the freeze is over and the member’s.  If a member does not comply with the freeze, or if a member cancels the member’s membership (whether by affirmative cancellation, nonpayment or otherwise), or if a member’s membership is terminated by C4C for any reason, the member will forfeit Program enrollment and benefits, and may later reapply for C4C membership at then current C4C membership rates, subject to any applicable terms, conditions, limitations and restrictions.  Freezes and cancellations are based on a member’s billing cycle; C4C does not prorate months of membership.

• Unlimited Groups/Classes; Surcharges.  Unlimited participation in open group classes at any C4C studio.  Note that certain markets have per visit surcharges associated with visiting member studio use which are in addition to a member’s normal monthly payment.

• Discounted Programs.   A twenty percent (20%) discount on all Special Programs & Events at any C4C Studio.  

• Retail Discount.  A ten percent (10%) discount on any retail merchandise purchase at any C4C studio, subject to any restrictions imposed by applicable manufacturers.  Discount does not apply to taxes or other assessments.

• Special Offers.  Special classes and offers that may be offered from time to time by C4C locations for Artful Wellness members.

 

Discounted Artful Wellness Member Program  - Additional Terms and Benefits

Discounted Artful Wellness Member Program (“Discounted Program”) is a special category of the Program available only to new members of C4C’s new studios who respond to an offer from C4C to enroll in the Program with a discounted membership fee of twenty present (20%) off the then market rate of the Program.   Members are subject to the general Program Rules, and additional terms and conditions below):

 

• Benefits.     

  • Discounted Membership Fee.    The Discounted Program is a discounted monthly fee as set forth in the invitation for the applicable new studio.  Due to the discounted rate for members, a first month discounted membership as is available for general Artful Wellness members is not a benefit of the Discounted Program.  Membership freezes and cancellations are subject to the terms described in Program benefits above.  There is no activation fee and no minimum commitment.  Prices are subject to applicable taxes, assessments and, as described above, C4C surcharges.  Membership is nontransferable.

• Eligibility.  Eligible participants in the Discounted Program are individuals who (a) have not had a C4C Artful Wellness membership of any kind (whether a standard Artful Wellness membership, a discounted membership rate or otherwise) in the four (4) months prior to enrollment for a Discounted Program membership at the applicable new C4C studio, (b) enroll for a monthly autopay C4C membership using a valid debit or credit card for a new, to-be-opened, C4C studio at which memberships for such studio are offered by C4C, and (c) are residents of the state in which the C4C studio is located.  As part of enrollment, all required membership documents, agreements and waivers must also be completed and submitted, including any Program forms.  Enrollment for Discounted Program membership for a new studio begins at the time C4C begins publicly distributing invitations for such new studio and ends when such studio opens.  To continue to be eligible for the Discounted Program membership, the participant’s membership and membership account must remain in good standing with C4C at all times, including, without limitation, by not terminating or changing the participant’s C4C membership, or allowing the participant’s C4C membership to be terminated, cancelled or lapse, and by maintaining payment by autopay for participant’s monthly membership fees.  Once eligibility ceases, a member will no longer have the Discounted Program membership or other Program benefits. C4C may also offer the Discounted Program membership through specific and/or restricted portals or methods (for example, specific C4C social media sites), and reserves the right to condition eligibility for such offers to participants utilizing such portal/method to accept or respond to such offer, and C4C may offer different, additional and/or modified benefits.  For such additional eligibility requirements and, if applicable, different, additional and/or modified benefits, consult the terms of the specific offer.

• Marketing Permission Waiver

  • I understand that C4C periodically takes photographs and videos of special events, classes, groups, programs and sessions in which I may be a part of. I hereby grant C4C irrevocable access to my image, voice and name via their website, all social media outlets, email mailings, postal mailings, community outreach, and news outlets. I give C4C my permission to use my likeness, name and image throughout their marketing and promotional materials from now until forever.

 

​Waiver and Release of All Claims and Assumption of Risk

 

Please read form carefully and be aware that in registering online/signing up and participating in the above identified programs/groups/activities, you will be expressly assuming the risk and legal liability and waiving and releasing all claims for injuries, damages or loss which you or your minor child/ward might sustain as a result of participating in any and all activities associated with said programs/groups/activities.

 

The Center for Creative Arts Therapy (herein collectively referred to as C4C) it’s programs and activities in a safe manner and holds the safety of participants in high regard. See foresee continually strives to reduce such risks and insists that all participants follow safety rules and instructions that are designed to protect the participant’s safety. However, participants and parents/guardians of minors registering for this program must recognize there is an inherent risk of injury when choosing to participate in activities/groups/programs.

 

You are solely responsible for determining if you or your minor child/Ward are physically fit and/or adequately skilled for the activities contemplated by this agreement. It is always advisable, especially if the participant is pregnant, suffers from an underlying medical condition, or has recently suffered an illness, injury or impairment, to consult a physician before undertaking any physical activity.

 

I recognize and acknowledge that there are certain risks of physical injury to participants in these programs/activities, and I voluntarily agreed to assume the full risk of any and all injuries, damages or losses, regardless of severity, that my minor child/ward or I may sustain as a result of said participation. I further agree to waive and relinquish all claims I or my minor child/ward may have (or accrue to me or my child/ward) as a result of participating in these programs/groups/activities against C4C, including its officials, agents, volunteers and employees.

 

I have read and fully understand the waiver and release of all claims on this page and the refund policy. This waiver is completed and agreed to of my own free will. All adult participants must sign one parent or custodial parent or guardian must sign a participant is under the age of 18 years.

 

Restrictions

 

In addition to the other limitations and restrictions set forth in these Program Rules, the following additional limitations and restrictions also apply.  Program benefits are nontransferable and may only be used by the applicable member, and only so long as the individual is in good standing with C4C.  Program benefits have no cash value, and are not redeemable for cash or any substitute.  Program benefits cannot be combined with any other offer, coupon or promotion, including any other promotional offers available to Program participants through the Program.  Program benefits cannot be applied to a prior purchase.

Changes to the Program or Benefits

 

We have the right at any time, in whole or in part, with or without giving you prior notice, to:

 

• change these Program Rules;    

• end the Program and/or any specific Program benefit;

• change, add or eliminate any participating locations or Program benefits;

• change any requirements for initial or continuing eligibility;

• change any other feature of the Program;

• offer Artful Wellness membership or other program memberships through specific and/or restricted portals and methods, and offer different, additional and/or modified benefits, terms and conditions for participants accepting through such portals or methods. The foregoing actions may be taken even though such actions may affect benefits in the Program or your participation in the Program.  We may make such changes and update these Program Rules at any time.  If we do change the Program Rules, then the revised Program Rules will be substituted.  A participant’s continued participation in the Program constitutes the member’s acceptance of any changes to these Program Rules.  Consult the Program Rules for terms and conditions then applicable to the Program.

Health Policy

​

Providing a healthy environment for all participants in C4C is a priority to the organization. Therefore, C4C reserve the right to limit attendance at groups/series/events to participants, determined at our discretion, not to be a health risk to others. Participants suffering from infectious diseases, open wounds, persistent coughs or colds or other potentially transmissible health conditions are to cancel groups.

 

Past Due Accounts

​

Delinquent accounts will be charged a 7% fee following each semester/summer. C4C reserves the right to suspend or discontinue groups of any customer with a past due account.

​

Weather cancellations

​

C4C does not necessarily follow public school cancellations. When weather conditions are questionable, cancellation notification will be as follows: Classes—check C4C website at c4creativeartstherapy.com.

Interpretation

​

C4C reserves the right to interpret and apply these Program Rules.  All determinations by C4C shall be final and conclusive in each case.  Our failure to enforce any term of these Program Rules shall not constitute a waiver of that provision or any other provision of these Program Rules.
